Leave Transfer Program Instructions

PURPOSE:  To ensure all Fort Gordon activities comply with the requirements involved for the Fort Gordon Voluntary Leave Transfer Program (VLTP)

  Application to Become a Leave Recipient Under the Voluntary Leave Transfer Program

 a.      The employee requesting admission will submit an Application to Become a Leave Recipient Under the Voluntary Leave Transfer Program (http://www.opm.gov/forms/html/opm.asp) OPM 630 as well as supporting medical documentation to the attention of the VLTP Program Coordinator, Sandra Taylor (Fax 706-791-5595).

b.      Upon the VLTP Program Coordinator approving the application to become a leave recipient under the VLTP, Coordinator will fax the OPM 630 and  the supporting documentation to the Customer Service Representative for input into DCPS:

  Clara Huggins, Customer Service Representative for IMA/TRADOC to (706) 791-2942

  Sharon Mister, Customer Service Representative for the EAMC to (706) 787-3292  

Request to Donate Annual Leave to Leave Recipient Under the Voluntary Leave Transfer Program (Within Agency)  

  1. The employee requesting to donate Leave will submit a Request to Donate Annual Leave to Leave Recipient Under the Voluntary Leave Transfer Program (Within Agency) (http://www.opm.gov/forms/html/opm.asp) OPM 630A to the attention of the VLTP Program Coordinator, Sandra Taylor (Fax 706-791-5595).

b.      The VLTP Program Coordinator will fax the OPM 630A and fax the supporting documentation to the Customer Service Representative for input into DCPS:

  Sharon Mister, Customer Service Representative for the EAMC to (706) 787-9061
