How to Complete the Hometown news Release Information Form

A Hometown News Release is only as good as the form it’s submitted on. It’s like most anything – the final product will only be as good as the raw material you have to work with. In this case, that ‘raw material’ is the DD Form 2266. The following is a close-up look at each block of the DD Form 2266 and what needs to be completed when putting together the ‘perfect’ form.
STEP 1: SERVICE INFORMATION
Last FIVE (5) of SSN: Note that this is asking for the last five digits of the Social Security number. This information is a must in order to release the form. It is used solely to track the form through the system. It is not released as part of the story.
Last Name
First Name
MI: Type your middle initial. If you don't have a middle name, leave blank.
Sex: Select M for male or F for female.
Pay Grade: In the drop-down window, click on your pay grade.
Service Branch: Click on the appropriate branch in the drop-down window.
Status: Click on the appropriate status in the drop-down window.
MOS/AFSC: List your Military Occupational Specialty or Air Force Specialty Code.
Full Job Title: List your complete job title.
Years of Service: Click on the appropriate number in the drop-down window.
Home Unit: Ignore - this is for Hometown News Center use only.
Home Post/Base (not APO): Ignore - this is for Hometown News Center use only.
City: Ignore - this is for Hometown News Center use only.
State or Country: Ignore - this is for Hometown News Center use only.
STEP 2: RELATIVE INFORMATION
For each relative or set of relatives, do the following:
Relative Name: List the family member or family members by complete name.
Relationship to You: Click on the appropriate relationship from the drop-down window.
Street Address: List the complete address.
City: List the city.
State: Click on the appropriate state in the drop-down window.
Zip Code: List the Zip code.
Enter Spouse's Name Here if Married: As the note says, if a spouse is listed here, in-law information must be included in one of the blocks for relatives.
STEP 3: EDUCATION INFORMATION
High School Graduated From: List the full name of the high school where you graduated.
Grad Year: Click on the appropriate year.
High School City: List the city where the high school you graduated from is located.
State: List the state where the high school you graduated from is located.
ZIP: List the Zip code of the high school where you graduated.
College School Graduated From: List the full name of the college where you graduated.
Grad Year: Click on the appropriate year.
College City: List the city where the college you graduated from is located.
State: List the state where the college you graduated from is located.
Repeat the "College Graduated From" directions for the next line, if appropriate.
STEP 4: RELEASE INFORMATION
Type of Hometown News Story: Indicate the reason you're submitting a Hometown News Release, such as a promotion, the receipt of an award, graduation or completion of training, etc.
Event Date: List the date of the promotion, award, graduation, etc.
Your Phone Number: By listing a current telephone number where you can be reached, you give the Hometown News Center staff the opportunity to contact you if they have questions about your release.
Your e-mail Address: By listing a current e-mail address where you can be reached, you give the Hometown News Center staff the opportunity to contact you if they have questions about your release.
Comments: Complete this section only if you need to provide additional information necessary to process the news release form.
STEP 5: PHOTOS AND OTHER ATTACHMENTS
Photographs, award citations and other documents can be attached to your Hometown Release Form to provide more information about the event or for possibly publication with the information from your form. Follow the directions on the form to attach documents.
